<p>Duane Costelow II, 30, of Indianapolis, died Saturday, July 23, 2016.</p>
<p>Survivors include his father, Duane (RaJeana) Costelow, mother, Dorothy Adams; sister, Dawn Costelow; and daughters, Jaicelyn Morgan and Aubrey Mae Costelow.</p>
<p>Memorial visitation will be 11 a.m. until the time of service at 1 p.m. Thursday at G.H. Herrmann Madison Avenue Funeral Home, 5141 Madison Ave., Indianapolis.</p>
<p>Memorial contributions may be made to the family in care of the funeral home.</p>
