<p>INDIANAPOLIS</p>
<p>Rose Ann Phelps, 65, of Indianapolis, died Sunday, Sept. 4, 2016, at Franciscan St. Francis Hospital in Indianapolis.</p>
<p>Survivors include her husband, Ken Phelps; sons, Brian (Jennifer) Phelps, Daniel (Stacey) Phelps; three grandchildren and three siblings, Peggy (David) Smith, Joe Page and David (Amanda) Page.</p>
<p>A funeral service will be at 1 p.m. Friday at Calvary Baptist Church, 200 Sunset Blvd, Greenwood. Visitation will be from 11 a.m. until service time Friday at the church. Entombment will follow at Forest Lawn Memory Gardens.</p>
<p>Memorial contributions may be made to Calvary Baptist Church.</p>
<p>Singleton Community Mortuary and Memorial Center is handling the arrangements.</p>
