<p>MARTINSVILLE</p>
<p>Tammy Jo Fields, 48, of Martinsville, formerly of Brown County and Center Grove, died on Friday, Dec. 2, 2016.</p>
<p>Survivors include her companion, Clint Frazee; children, Sierra Frazee and Josh Bandy; three grandchildren; parents, Robert E. and Donna J. Fields; and siblings, Wes Fields, Kathy Fields and Christian Fields.</p>
<p>A celebration of life will be scheduled in the spring at Lake Lemon.</p>
<p>Arrangements entrusted to G.H. Herrmann Greenwood Funeral Home at The Gardens at Olive Branch.</p>
